Healthcare facilities are the most demanding environment in fire inspection. The systems are more complex, the documentation requirements are more rigorous, the regulatory oversight comes from multiple directions simultaneously, and the consequences of non-compliance — for the facility and potentially for the contractor — are more severe than in any other building type.

Why Healthcare Is Different

The fundamental difference between healthcare and every other building type is the defend-in-place strategy. Hospitals cannot evacuate patients in ICUs, operating rooms, or intensive care settings. The fire response is horizontal evacuation — moving patients to a safe smoke compartment — not vertical evacuation out of the building.

The Accreditation Landscape

Most hospitals are accredited by one of three organizations, each of which imposes its own fire safety documentation requirements:

The Joint Commission

The largest accreditor, covering approximately 4,000+ hospitals plus behavioral health facilities, home health agencies, nursing homes, and labs. Surveys every three years, unannounced. Uses EC (Environment of Care) standards with EP (Elements of Performance) codes. The relevant fire protection standard is EC.02.03.05.

DNV Healthcare

The second largest, accrediting approximately 600 US hospitals. Surveys annually, unannounced. Uses NIAHO (National Integrated Accreditation for Healthcare Organizations) standards with SR codes. The relevant fire protection standard is PE.2, SR.10. DNV integrates ISO 9001 quality management requirements alongside CMS Conditions of Participation.

HFAP (Healthcare Facilities Accreditation Program)

Approximately 400 facilities, managed by the AOA Bureau of Healthcare Facilities Accreditation. Surveys every three years. Smaller market share but present in specific regions and facility types.

A facility not accredited by any of these organizations still must comply with NFPA codes and CMS Conditions of Participation if it accepts Medicare or Medicaid — but it doesn't have the additional accreditation reporting layer.

The NFPA Standards Stack

Healthcare fire inspection involves multiple NFPA standards simultaneously. Here's what each covers and why it matters in healthcare:

NFPA 101 — Life Safety Code

The overarching framework for fire and life safety in buildings. Healthcare occupancy chapters govern fire doors, smoke compartmentation, corridor widths, egress paths, emergency lighting, and exit signs. NFPA 72 — National Fire Alarm and Signaling Code

Governs fire alarm system inspection, testing, and maintenance. In healthcare, this includes the 10%/year smoke detector sampling cycle, emergency voice/alarm communication systems, and the zone-specific notification requirements that support the defend-in-place strategy. NFPA 25 — Water-Based Fire Protection Systems

Governs sprinkler system, standpipe, fire pump, and backflow preventer inspection, testing, and maintenance. Healthcare facilities typically have complex water-based systems with multiple risers, zones, and connection points. NFPA 25 inspection frequency requirements include weekly, quarterly, and annual testing cycles.

NFPA 10 — Fire Extinguishers

Monthly visual inspection and annual service for all portable fire extinguishers. Healthcare facilities have large numbers of extinguishers distributed throughout clinical and non-clinical areas.

NFPA 96 — Kitchen Hood Suppression

Applies to any hospital with a commercial kitchen — which includes virtually all hospitals with patient food service operations. Semiannual inspection of hood suppression systems.

NFPA 855 — Energy Storage Systems

NFPA 99 — Medical-Gas Cylinder Storage (Chapter 11)

Hospitals store oxygen and other medical gases in cylinders and containers, and NFPA 99 Chapter 11 governs how those storage areas are built, separated, secured, and signed — fire separation, oxidizer-from-flammable separation, ignition-source control, and liquid-oxygen storage. NFPA 110 — Emergency & Standby Power (EPSS)

The defend-in-place strategy depends on power, so CMS and the accreditation bodies expect documented emergency-generator testing. NFPA 110 Chapter 8 covers the recurring ITM: weekly/monthly program verification, the monthly load test with run-time and load capture, annual load-bank and fuel-quality testing, and transfer-switch operation. Report Format Requirements

This is the area where most inspection contractors fall short. The NFPA standards define what to inspect. The accreditation bodies define how to present the results.

Joint Commission — EC.02.03.05 format

Reports must be organized by EC standard and EP code. Each inspection item maps to a specific EP. The report shows pass/fail/N/A counts per EP and overall compliance status. DNV — NIAHO PE.2 format

Reports reference NIAHO SR codes instead of EC codes. Structure: cover sheet, compliance summary, inspection results by system, signature page. No EP codes. Simpler format than Joint Commission but still requires intentional organization around NIAHO standards. DNV vs Joint Commission comparison →

Standard NFPA format

For non-accredited facilities, a standard NFPA inspection report organized by system and device type is appropriate. This is the default format for commercial and non-healthcare inspections.

The practical implication: If you service both accredited and non-accredited facilities, you need to produce three different report formats from the same inspection data. What Continuous Compliance Looks Like
